Patient Registration


To Pre-register online with the practice please click on the link below and select the Register with the Apples Medical Centre option.

Online request for pre-registration with the practice

Please note If you do not email or present to the surgery with your GMS form you will not be registered with the surgery, the online form is only pre-registation. If you do not present to or email the surgery with valid photo ID and proof of address within two weeks of registering, your registration will be cancelled.

 

Online:
  1. Complete the online GMS1 form in as much detail as possible and click the submit button when you have finished; then print-to-PDF, electronically sign it and e-mail it to applesmedcentre@nhs.net, DO NOT post your GMS form. If you prefer you can download a GMS form here to complete by hand or on your computer, which you can also e-mail to us: GMS1 form
  2. After completing the registration form, please complete our New Patient Health Questionnaire
  3. After completing your GMS form and New Patient Questionnire you MUST e-mail your GMS form and a scanned image or photograph of a valid form of photo identification (e.g. Driving License or Passport) and proof of address (e.g. utility bill, council tax statement etc.).
In person:
  1. Complete the online GMS1 form in as much detail as possible and click the submit button when you have finished; then print the form and sign it, DO NOT post your GMS form. If you prefer you can download a GMS form here to complete by hand: GMS1 form
  2. After completing the registration form, please complete our New Patient Health Questionnaire (The online form is automatically e-mailed to us, you do not need to print)
  3. After completing your GMS form and New Patient Questionnire you MUST present to the surgery with your printed GMS form and a valid form of photo identification (e.g. Driving License or Passport) and proof of address (e.g. utility bill, council tax statement etc.)

 

Once we have fully registered you, your may be offered an appointment in the surgery for a new patient health check with the practice nurse, or doctor if you need your medication reviewing; or a telephone review.

At the time of registering we will also ask if you would like to register for the SystmOnline service. This online self-service facility enables you to order repeat medications and book appointments from the comfort of your own home using laptop or desktop computer, or when you're on the move via a tablet or smart phone.
